How Do Food Allergies Develop: Causes and Risk Factors

Food allergies develop when your immune system mistakenly identifies a harmless food protein as a threat and builds a defense against it. This process, called sensitization, can happen through the skin, the gut, or both, and it’s shaped by genetics, the timing of food exposure, and the trillions of bacteria living in your digestive tract. About 6.7% of U.S. adults had a diagnosed food allergy in 2024, and the rate is higher in younger adults and children, suggesting that something about modern life is pushing immune systems toward overreaction.

How Sensitization Works

Your immune system has a specialized antibody called IgE whose normal job is to detect parasites and other genuine threats. When it identifies something dangerous, it signals the body to release histamine, which triggers inflammation to fight the invader. In a food allergy, this system misfires. The immune system produces IgE antibodies targeted at a specific food protein, like a protein in peanuts or eggs, even though that protein is completely harmless.

The first time this happens, you won’t notice anything. Your body is quietly building its arsenal, producing IgE antibodies and attaching them to immune cells throughout your tissues. It’s only on the second or subsequent exposure that those primed immune cells recognize the food protein, release a flood of histamine, and produce the symptoms you associate with an allergic reaction: hives, swelling, difficulty breathing, or in severe cases, anaphylaxis. The reaction typically hits within minutes to a few hours.

Where Exposure Happens Matters

One of the most important discoveries in allergy research is that the route of first exposure, whether through the skin or through the mouth, can determine whether your immune system learns to attack a food protein or tolerate it. This is known as the dual allergen exposure hypothesis, and it has reshaped how doctors think about allergy prevention.

When food proteins enter through intact, healthy gut lining during digestion, the immune system tends to recognize them as safe. The gut is designed to encounter foreign proteins constantly, and it has built-in mechanisms to teach immune cells tolerance. But when food proteins slip through damaged or inflamed skin, the response is very different. Inflamed skin cells release alarm signals that activate the immune system’s attack pathway, priming it to produce IgE antibodies against whatever protein came through the barrier.

This is why eczema, especially in infants, is one of the strongest risk factors for developing food allergies. A baby with cracked, inflamed skin on their face or hands might encounter trace amounts of peanut or egg protein from a parent’s hands, a kitchen surface, or household dust. Those proteins enter through the broken skin barrier, and the immune system treats them as invaders. Animal studies confirm this pattern: applying allergens to intact skin produces minimal immune response, while applying the same allergens to damaged skin triggers a surge in IgE production.

The Genetics of a Leaky Skin Barrier

A protein called filaggrin is essential for building a strong, waterproof outer layer of skin. It bundles structural fibers together and forms the tough envelope of skin cells, and its breakdown products act as a natural moisturizer that keeps skin flexible and intact. Some people carry mutations in the gene that produces filaggrin, and those mutations are the strongest known genetic risk factor for eczema.

The connection to food allergies is direct. Research published in The Journal of Allergy and Clinical Immunology found that filaggrin mutations increase the risk of food allergy independent of whether the person has eczema. These mutations also make allergies harder to outgrow. Children with filaggrin mutations were more likely to have persistent egg and cow’s milk allergies, the two most common childhood food allergies, rather than the pattern of gradual resolution that many children experience. In animal models, the impaired skin barrier from defective filaggrin allowed allergens to penetrate the skin and trigger systemic sensitization, leading to allergic reactions in distant organs like the lungs and gut.

Your Gut Bacteria Play a Protective Role

The community of bacteria in your gut has a powerful influence on whether your immune system develops tolerance to food proteins or reacts against them. Beneficial gut bacteria digest dietary fiber and produce short-chain fatty acids, including one called butyrate, that actively expand a population of regulatory immune cells. These regulatory cells act as peacekeepers, calming down the immune responses that would otherwise escalate into allergies.

Studies of infants have found that babies with lower levels of certain bacterial species in their stool at 3 to 6 months of age were more likely to become sensitized to food allergens. The specific bacteria that appeared protective, including species from the Clostridium and Dorea groups, are part of the normal gut ecosystem that develops during early life. Anything that disrupts this ecosystem during a critical window, such as antibiotic use, cesarean delivery, or a low-fiber diet, may leave the immune system without the bacterial signals it needs to learn tolerance.

Why Some Immune Systems Overreact

The immune system has different modes of response. One mode, driven by a class of cells called Th1 cells, handles bacteria and viruses. Another mode, driven by Th2 cells, handles parasites and, when overactivated, drives allergic reactions. In people who develop allergies, the Th2 pathway dominates when it shouldn’t.

Exposure to microbial products in the environment, particularly components of bacterial cell walls, plays a fundamental role in training the immune system away from the Th2 pathway during infancy. Children who grow up with less microbial exposure, in highly sanitized environments, in urban settings, or without early contact with diverse bacteria, are more likely to develop Th2-dominant immune responses. This helps explain why food allergies are more common in metropolitan areas (6.8%) than in nonmetropolitan areas (5.9%), and why allergy rates have climbed as modern hygiene has reduced children’s contact with environmental microbes.

Early Introduction Prevents Allergies

For decades, pediatric guidelines recommended delaying the introduction of allergenic foods like peanuts, eggs, and fish. That advice turned out to be exactly wrong. The landmark LEAP trial, funded by the National Institutes of Health, demonstrated that feeding peanut products to infants regularly from early in life reduced peanut allergy rates by 81% by age 5. Even more striking, follow-up data showed the protection lasted. Children who ate peanut products regularly from infancy through age 5 had a 71% lower rate of peanut allergy in adolescence, even after years of eating peanut however they pleased.

Current guidelines from the National Institute of Allergy and Infectious Diseases now recommend early introduction based on risk level:

  • High-risk infants (those with severe eczema, egg allergy, or both) should be introduced to peanut-containing foods as early as 4 to 6 months, after they’ve started other solid foods.
  • Moderate-risk infants (those with mild-to-moderate eczema) should start peanut-containing foods around 6 months.
  • Low-risk infants (no eczema or food allergy) can have peanut-containing foods introduced freely alongside other solids.

The logic connects directly back to the dual exposure hypothesis. Getting food proteins into the gut early, through eating, trains the immune system toward tolerance before skin exposure has a chance to trigger sensitization. The window matters because the infant immune system is still forming its categories of “safe” and “dangerous.” Once IgE antibodies are already in production, reversing that response is far harder.

Food Allergies Can Start at Any Age

Although most food allergies begin in childhood, adults develop new food allergies too. About 7.4% of adults aged 18 to 44 report a food allergy, and some of those emerge for the first time in adulthood. Shellfish allergy, in particular, commonly appears in adults who previously ate shellfish without any problem. The triggers for adult-onset food allergy are less well understood, but changes in gut bacteria composition, new environmental exposures, and shifts in immune regulation all likely contribute.

Women are more likely than men to have a food allergy (8.3% versus 5.1%), a gap that may reflect hormonal influences on immune function. The rate also varies by race: Black adults have the highest prevalence at 9.9%, compared with 6.4% for White adults, 5.5% for Asian adults, and 5.4% for Hispanic adults. These disparities likely reflect a combination of genetic, environmental, and socioeconomic factors rather than any single cause.
