Dermatologists commonly prescribe oral antibiotics, such as doxycycline or minocycline, to reduce bacteria and inflammation throughout the skin. These are often used in combination with topical treatments.
Oral retinoids, like isotretinoin, are highly effective for severe cystic acne, working by significantly reducing oil production and preventing pore clogging. Hormonal therapies, including certain oral contraceptives or spironolactone, can be beneficial for women by regulating androgen levels that contribute to acne. In-office procedures performed by a dermatologist, such as corticosteroid injections directly into individual cysts, can rapidly reduce inflammation and pain. Incision and drainage may also be performed to remove pus from a cyst. It is strongly advised against self-squeezing or picking these lesions, as this can worsen inflammation, spread infection, and increase the risk of permanent scarring.

A primary concern with cystic acne is its potential to cause permanent scarring due to the deep inflammation and tissue destruction. When cysts rupture, the profound inflammatory process can damage collagen and elastin fibers in the skin, leading to various types of scars, including atrophic (depressed) or hypertrophic (raised) marks. The severity of scarring can vary, but delaying treatment or physically manipulating the cysts increases this risk.
Beyond permanent textural changes, cystic acne can also result in post-inflammatory hyperpigmentation, which manifests as temporary dark spots on the skin after lesions heal. While these dark spots typically fade over time, true scarring is permanent.
