Bursitis is a condition marked by the inflammation of a bursa, which is a small, fluid-filled sac. These bursae function as cushions, reducing friction between bones, tendons, and muscles near joints, allowing for smooth and painless movement.
When a bursa becomes irritated or inflamed, it can swell and cause pain, tenderness, and limited range of motion in the adjacent joint. Common causes of bursitis include repetitive motion, prolonged pressure, injury, or infection. Bursitis frequently affects large joints that undergo repetitive stress, such as the shoulders, elbows, hips, and knees.
How Gout Causes Bursitis
Gout can directly cause bursitis through the deposition of uric acid crystals. The same monosodium urate crystals responsible for gout attacks within joints can also accumulate inside or around bursae, leading to their inflammation. This particular form of bursal inflammation is known as “gouty bursitis,” where the needle-shaped urate crystals irritate the bursal lining, causing the bursa to become swollen, painful, and red.
This crystal deposition commonly occurs in bursae situated near joints prone to gout attacks. Examples include the olecranon bursa at the elbow and the prepatellar bursa at the knee. In instances of long-standing, untreated gout, larger deposits of these urate crystals, called tophi, can form within bursae and other soft tissues. These tophi, when present in a bursa, lead to “tophaceous bursitis,” contributing to persistent inflammation.
Diagnosing Gout-Related Bursitis
Distinguishing gout-related bursitis from other causes requires a comprehensive diagnostic approach. Healthcare providers typically begin with a thorough review of the patient’s medical history, including any prior gout attacks, and a physical examination of the affected bursa. The appearance of sudden, severe pain, swelling, and redness in a bursa, especially if there’s a history of gout, can raise suspicion.
Imaging techniques, such as ultrasound, X-rays, or MRI, can help visualize the inflamed bursa and sometimes reveal uric acid crystal deposits. However, the definitive diagnosis of gouty bursitis relies on aspirating fluid directly from the inflamed bursa. This bursal fluid is then examined under a microscope for the presence of monosodium urate crystals. Blood tests measuring uric acid levels can support the diagnosis, but elevated levels alone are not conclusive, as some individuals with high uric acid may not have gout, and some with gout may have normal levels during an attack.
Management and Prevention
Managing gout-related bursitis primarily involves addressing the underlying gout to reduce uric acid levels and prevent further crystal deposition. For acute flares, treatments aim to alleviate pain and inflammation rapidly. These include nonsteroidal anti-inflammatory drugs (NSAIDs), colchicine, and corticosteroids, which can be administered orally or sometimes directly into the affected bursa. Applying ice and resting the affected joint can also help reduce swelling and discomfort.
Long-term management focuses on preventing future attacks by lowering uric acid levels. This often involves daily medications like allopurinol or febuxostat, which reduce uric acid production. Lifestyle modifications also play a role, including maintaining a healthy weight, staying well-hydrated, and adjusting dietary habits to limit purine-rich foods and alcohol. In rare and severe cases where large tophi in bursae cause significant problems, surgical removal might be considered.
