Portuguese neurologist Egas Moniz theorized that certain mental illnesses, such as obsessive thoughts or fixed delusions, stemmed from problematic brain circuits in the frontal lobes. He believed that severing these neural pathways could disrupt these patterns and alleviate severe psychiatric symptoms. Moniz’s hypothesis suggested that by disconnecting these circuits, the brain could functionally adapt, leading to an improvement in the patient’s condition. This theoretical basis provided the perceived scientific justification for the surgical intervention.
How Lobotomies Were Performed
The techniques used for lobotomies evolved over time, starting with more invasive methods. Egas Moniz, alongside his colleague Almeida Lima, performed the first “leucotomy” in 1935, which involved drilling holes into the skull. Through these holes, they initially injected alcohol into the frontal lobes to destroy connecting fibers. Moniz later refined his approach using a specialized instrument called a leucotome, which featured a retractable wire loop to cut cores in the white matter of each hemisphere.
A significant shift occurred with the transorbital lobotomy, popularized by American neurologist Walter Freeman. Developed in 1945, this method was less invasive, involving the insertion of a pick-like instrument through the thin bone at the back of the eye sockets directly into the frontal lobes. Freeman would then manipulate the instrument to sever neural connections, often without general anesthesia, relying on electroshock to induce unconsciousness. This technique aimed to disconnect pathways.
Why Lobotomies Became Widespread
Lobotomies gained widespread adoption due to the desperate state of mental health care during the mid-20th century. Psychiatric institutions were severely overcrowded, and there was a significant lack of effective treatments for individuals suffering from severe mental illnesses. The procedure offered a perceived solution, providing hope for patients, their families, and clinicians struggling with intractable conditions. Initial reports highlighted “successes” in calming agitated patients and reducing severe symptoms, even if these improvements were temporary or limited in scope.
Prominent figures like Walter Freeman actively promoted the procedure, making it seem more accessible. Freeman’s simplified transorbital method, which did not require a neurosurgeon or a sterile operating room, contributed to its rapid dissemination. By 1951, nearly 20,000 lobotomies had been performed in the United States, with estimates reaching 50,000 procedures by 1952. This widespread acceptance reflected the urgent need for any intervention that offered relief in a challenging medical landscape.
The Shift Away from Lobotomy
The practice of lobotomy began to decline significantly in the 1950s as growing concerns emerged regarding its efficacy and ethical implications. Patients often experienced severe and irreversible side effects, including personality changes, cognitive impairment, and a general reduction in emotional depth and initiative. These outcomes led to increased professional criticism and public awareness of the procedure’s negative consequences.
A key factor in the abandonment of lobotomies was the development of psychopharmacology. In the early 1950s, the introduction of the first effective antipsychotic medication, chlorpromazine, marked a turning point. Synthesized in 1950 and available by 1952, chlorpromazine offered a less invasive and often more effective alternative for managing psychiatric symptoms like psychosis and agitation. This pharmaceutical breakthrough provided a safer and more humane treatment option, rendering lobotomy largely obsolete. The combination of ethical concerns, unpredictable outcomes, and the advent of effective drug therapies led to the rapid decline and eventual abandonment of lobotomy as a mainstream medical practice.
