The breakdown products are smaller fragments, such as oligosaccharides and disaccharides, which can then be more easily absorbed and eliminated by the body.

Hyaluronidase serves as a “spreading factor” in medical contexts, enhancing the diffusion and absorption of various injected drugs. For instance, it is co-administered with local anesthetics in procedures like ophthalmic surgery to promote faster and wider dispersion of the anesthetic agent. This can lead to increased analgesic efficacy and a larger anesthetized area.
The enzyme is also utilized in hypodermoclysis, a method for subcutaneous fluid administration, to facilitate the absorption of large volumes of fluids for hydration. In cases of extravasation, where injected fluids or medications leak from a vein into surrounding tissue, hyaluronidase can be administered to reduce tissue damage by aiding the dispersion of the extravasated substance. It is also used as an adjunct in subcutaneous urography to improve the absorption of radiopaque agents, allowing for better imaging.
Uses in Cosmetic Procedures
In cosmetic dermatology, hyaluronidase is widely used to manage complications or undesirable outcomes associated with hyaluronic acid-based dermal fillers. These fillers are popular for volume replacement and soft tissue augmentation. Hyaluronidase can effectively dissolve these cross-linked hyaluronic acid fillers.
This application is important for correcting issues such as overfilling, asymmetry, or the formation of lumps and nodules. A more serious complication hyaluronidase addresses is vascular occlusion, which occurs when filler is inadvertently injected into a blood vessel. Prompt administration of hyaluronidase can prevent skin necrosis and other tissue damage by breaking down the filler blocking blood flow.
Safety and Considerations
While generally well-tolerated, hyaluronidase carries potential side effects. Common local reactions at the injection site include pain, swelling, itching, or redness. More serious, though rare, adverse effects include hypersensitivity reactions and anaphylaxis, a severe allergic response.
Contraindications for hyaluronidase include a known hypersensitivity to the enzyme or its components. It should not be injected into or around infected or inflamed tissues, as this could spread a localized infection.
