Examination of the throat may reveal red and swollen tonsils, often displaying white patches or streaks of pus. Tiny red spots, known as petechiae, might also be visible on the roof of the mouth. Swollen and tender lymph nodes in the neck are common indicators.

Diagnosing a bacterial sore throat involves a visit to a healthcare professional. The most common diagnostic tools are the rapid strep test and the throat culture. A rapid strep test detects bacterial antigens within minutes from a throat swab and is highly accurate.

If the rapid test yields a negative result but a bacterial infection is still suspected, a throat culture may be performed. While a throat culture takes longer, it offers a more definitive diagnosis. Once a bacterial infection is confirmed, antibiotics are the standard treatment. Antibiotics eradicate the bacteria, unlike viral infections which do not respond to them. It is important to complete the entire course of antibiotics, even if symptoms improve, to ensure the infection is fully cleared and to prevent complications.

Properly diagnosing and treating a bacterial sore throat prevents complications. Untreated strep throat can lead to serious health issues. These include rheumatic fever, an inflammatory condition that can affect the heart, joints, brain, and skin, potentially leading to permanent heart damage.

Other concerns are post-streptococcal glomerulonephritis, which involves inflammation of the kidneys, and a peritonsillar abscess, a collection of pus near the tonsils, causing severe pain and difficulty swallowing or breathing.
