Monoarthritis: Causes, Symptoms, and Treatment

Monoarthritis is a condition marked by inflammation affecting a single joint in the body. The term “monoarthritis” combines “mono,” meaning one, with “arthritis,” which refers to joint inflammation. This condition can manifest suddenly, known as acute monoarthritis, and requires prompt medical evaluation to determine its underlying cause.

Understanding the Causes

Monoarthritis can stem from various factors, including infections, crystal deposits, injuries, and certain degenerative or inflammatory conditions. Septic arthritis, an infection within a joint, is a serious condition requiring immediate attention. Bacteria, such as Staphylococcus aureus or Neisseria gonorrhoeae, can cause this condition, often affecting larger joints like the knee or hip.

Crystal-induced arthritis is another frequent cause, primarily involving gout and pseudogout. Gout occurs when uric acid crystals accumulate in a joint, commonly the big toe, leading to intense pain and swelling. Pseudogout, on the other hand, results from calcium pyrophosphate crystals depositing in joints, frequently affecting the knee.

Traumatic injuries, such as sprains, fractures, or internal joint damage, can also lead to single-joint inflammation. Osteoarthritis, a degenerative joint disease often associated with aging, may initially present in a single joint. Systemic inflammatory conditions, like rheumatoid arthritis or psoriatic arthritis, though typically affecting multiple joints, can sometimes begin with inflammation in just one joint.

Recognizing the Symptoms

Monoarthritis is characterized by observable signs and sensations localized to one joint. Patients typically experience localized pain, which often intensifies with movement. The affected joint usually appears swollen, indicating an accumulation of fluid or tissue inflammation.

The skin over the inflamed joint may feel warm to the touch and can also show visible redness. Individuals often report a limited range of motion, making it difficult to move the affected joint fully or without discomfort. These symptoms are confined to a single joint, which helps differentiate monoarthritis from conditions affecting multiple joints.

How Monoarthritis Is Diagnosed

A medical professional will begin by taking a detailed medical history, asking about the onset and nature of symptoms, any recent injuries, or other health conditions. This is followed by a physical examination to assess the affected joint for swelling, warmth, redness, and to evaluate its range of motion.

Joint fluid analysis, or arthrocentesis, is a valuable diagnostic tool for monoarthritis. During this procedure, a doctor draws fluid directly from the inflamed joint using a needle. This synovial fluid is then analyzed for various indicators, including the white blood cell count, the presence of specific crystals (such as uric acid for gout or calcium pyrophosphate for pseudogout), and for bacterial infection through culture.

Blood tests are also performed to support the diagnosis. These tests may include general inflammatory markers like erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP). Specific blood tests, such as uric acid levels, can help confirm gout, while certain antibodies might be checked if a systemic inflammatory disease is suspected. Imaging studies, such as X-rays, are used to assess for bone damage or changes indicative of osteoarthritis. Ultrasound or magnetic resonance imaging (MRI) may provide more detailed views of soft tissues, cartilage, and joint effusions.

Treatment Approaches

Treating monoarthritis involves general measures to manage symptoms and specific interventions targeting the underlying cause. Initial approaches include resting the affected joint, applying ice to reduce swelling and pain, and elevating the limb. Over-the-counter pain relievers, such as nonsteroidal anti-inflammatory drugs (NSAIDs) or acetaminophen, can help alleviate discomfort.

For infectious causes like septic arthritis, immediate administration of antibiotics is necessary to prevent joint destruction. The specific antibiotic chosen depends on the identified bacteria. In cases of crystal-induced arthritis, such as gout or pseudogout, treatment focuses on reducing inflammation during acute attacks using medications like NSAIDs, colchicine, or corticosteroids. Long-term management for gout may involve medications like allopurinol to lower uric acid levels and prevent future attacks.

If monoarthritis is an early manifestation of a systemic inflammatory condition, corticosteroids, either oral or injected directly into the joint, may be used to control inflammation. Disease-modifying antirheumatic drugs (DMARDs) may be considered for long-term management. For traumatic injuries, treatment can range from immobilization and physical therapy to surgical intervention, depending on the severity and type of injury. Following initial treatment, physical therapy is often recommended to help regain full joint function and mobility.
