Once a Lantus vial has been punctured or a SoloStar pen used for the first time, its storage requirements change, and its shelf life begins. After initial use, Lantus is stable for 28 days when stored at room temperature, typically below 86°F (30°C).

During this period, refrigeration is not necessary for opened Lantus pens and they should not be returned to the refrigerator. Cold insulin can be painful to inject and may affect the pen’s mechanism.

Insulin compromised by improper storage or exceeding its shelf life may show visual signs of degradation. Lantus should always appear clear and colorless; any cloudiness, discoloration, or particles indicates it may no longer be effective and should not be used.

Using compromised insulin carries significant risks, primarily the inability to manage blood sugar. Ineffective insulin can lead to hyperglycemia, a condition where blood glucose levels are dangerously high. This lack of blood sugar control can lead to acute complications or long-term health issues.

It is important to immediately discard any Lantus that shows these visual signs or has been exposed to extreme temperatures, such as freezing or excessive heat, even if it is within its 28-day open period.

Safe Disposal of Lantus

Proper disposal of Lantus pens, vials, and needles is important for safety and the environment. Used pen needles should always be placed immediately into a designated sharps container to prevent accidental needle sticks. These containers safely hold sharp medical waste, reducing injury risk.

For disposal of expired or compromised insulin, or empty vials and pens, follow local regulations. Many communities offer medication take-back programs or specific instructions for household disposal, such as mixing the insulin with an unappealing substance like coffee grounds or cat litter, placing it in a sealed bag, and then discarding it in household trash. Insulin should never be flushed down the toilet or poured down a drain.
