Scar tissue is the body’s repair mechanism following an injury to the skin. When the skin’s deeper layers are damaged, the body initiates a rapid healing process, resulting in the formation of a scar. This tissue does soften over time, a predictable part of the biological process known as scar maturation or remodeling. This transformation from a firm, red, and often raised initial mark to a softer, flatter, and paler line is a gradual journey dictated by cellular changes within the tissue itself.
The Biological Mechanism of Scar Softening
The initial firmness and thickness of a fresh scar are due to the rapid deposition of collagen to close the wound quickly. This initial “patch” is largely composed of type III collagen, which is laid down quickly and in a disorganized pattern, giving the new tissue its characteristic rigidity and red appearance. Fibroblasts, the primary cells responsible for producing this structural protein, work in the early phases of healing.
The softening process begins when the scar enters the remodeling phase, where the body starts to refine this emergency repair job. Special enzymes called matrix metalloproteinases (MMPs) are activated to break down the excess type III collagen fibers. Simultaneously, fibroblasts begin to synthesize the stronger, more organized type I collagen, which is the main structural protein found in unwounded skin.
This breakdown and replacement process is known as collagen remodeling. The overall mass of scar tissue decreases as excess cells and blood vessels undergo programmed cell death (apoptosis), resulting in a less dense and less vascularized—and therefore softer and paler—scar. The new type I collagen fibers are aligned under tension, giving the mature scar better tensile strength, though it will only achieve about 80% of the strength of the original skin.

The softening of a scar is the result of the final stage of wound healing, the maturation or remodeling phase. Wound healing is generally categorized into four overlapping stages: hemostasis, inflammation, proliferation, and maturation. While the first three stages are completed within a few weeks, the remodeling phase determines the scar’s final appearance.
This final phase typically begins around three weeks after the initial injury and can continue for 12 to 18 months. For deeper injuries, softening and fading can continue for up to two years. Significant softening, flattening, and a decrease in redness occur most noticeably during this extended period.
Factors That Influence Scar Texture and Appearance
The final texture and visibility of a scar are influenced by several factors, many of which are outside of a person’s control. Genetics play a role, as some individuals are predisposed to developing pathological scars, such as keloids or hypertrophic scars, characterized by excessive collagen production. The location of the injury also matters; scars over areas of high skin tension, like joints or the chest, are more prone to becoming thicker and raised due to constant mechanical stress.
The type and depth of the initial injury are also determinants of the outcome. A clean surgical incision generally heals better than a jagged cut or a severe burn, which destroys more surrounding tissue architecture. Age is another factor, as younger skin tends to have a more vigorous inflammatory response, which can sometimes lead to more prominent scarring.
Effective Methods for Scar Management
Individuals can take proactive steps to encourage natural softening and improve a scar’s appearance during the maturation phase. Scar massage is a recommended technique, which should begin only after the wound is fully closed and healed. Massaging the scar with firm pressure helps mechanically break down disorganized collagen fibers and increase blood flow to promote better remodeling.
Silicone products, available as sheets, gels, or tapes, are considered a primary non-invasive treatment for improving scar outcome. Silicone works by creating a semi-occlusive barrier over the scar, which helps hydrate the tissue and regulate collagen production. This leads to a flatter, softer result. Consistent use for many hours a day over several months is required for the best benefit.
Protecting the healing scar from the sun is important, as UV exposure can cause hyperpigmentation, making the scar permanently darker than the surrounding skin. Applying a broad-spectrum sunscreen is necessary when the scar cannot be covered by clothing. For scars that become excessively raised, red, or painful—such as hypertrophic scars or keloids—medical interventions like corticosteroid injections or laser treatments may be necessary to halt excessive collagen production.
